All permanent employees working 18.75 hours or more per week and their eligible dependent(s) may participate. Employees have 60 days from their date of hire or qualifying event to enroll. Dental care benefits are paid up to the scheduled allowances found in the Securian Group Dental Plan Booklet. Examples of qualifying life events include but are not limited to losing or obtaining other coverage, marriage, formation of a domestic partnership, birth, adoption or placement of adoption.
Covered treatment includes routine exams, dental x-rays, fillings, crowns and orthodontia. Weekly costs are $6.37 for single plan, $15.44 for two-person plan and $19.04 for family plan. Rate increases are effective in July. Open enrollment is held during June each year with an effective date of coverage for July 1. Payroll deductions are on a pre-tax federal, state and Medicare basis.
